Each academic program planning form requests detailed information required for SUNY
and/or the State Education Department and contains detailed instructions in the form
of notes and hyperlinks. Program forms are located off site. Please use the above
link for forms.
The Program Proposal is the formal request from a campus to have a new undergraduate academic program approved by the State University and subsequently registered by the State Education Department. The Program Proposal must be approved the University Provost after the presentation to the curriculum committee. Please allow time for this when planning new programs. The Program Proposal guidelines link below offers detailed information on the creation and submission of new program announcements and proposals.
A suggested course sequence must be submitted with the proposal. A sample is linked above. The college catalog has many more examples.
