Testing Times:
- Monday, Tuesday, Thursday and Friday 9:00am - 4:00pm by appointment only (Tests may not be start after 2:30 pm.)
- Wednesday 9:00am - 6:00pm by appointment only (Tests may not be start after 4:30 pm.)
- Please note that you must complete this form 1- 2 days prior to exam day. Weekend
days do not count. For Ex: if you need to take a test on Monday, then the form should
be completed by 4:00 pm on Friday.
