Submit Contract Request

If you need a formal agreement signed by an authorized campus signatory, please complete the required form. All required fields must be completed for your request to be reviewed. This applies to all agreements, including contracts, memoranda of understanding (MOUs), leases, revenue agreements, and similar documents.

Requests submitted without this form will not be reviewed.

If your request involves the procurement or use of software, the vendor must provide both a Higher Education Community Vendor Assessment Toolkit (HECVAT) and a Voluntary Product Accessibility Template (VPAT). These documents are reviewed by Information Technology (IT) and the Business and Fiscal Affairs Office to determine whether the proposed vendor meets SUNY’s accessibility standards as well as higher education security and safety requirements.
